Registration Forms

Please download and complete the forms below and bring them with you to registration night on Thursday, June 9th from 6:30 p.m. to 9:00 p.m. or Saturday, June 11th from 1:30 p.m. to 3:30 p.m. at Veteran's Park, 7600 NW 50th St, Lauderhill, FL 33351. A deposit of $100 will be collected along with the forms. If player is new to the TRAVEL PROGRAM, a birth certificate must accompany the registration forms.

Registration Fees:

U18/U19 Girls $995.00 per player

U8-U13 Boys & Girls $850.00 per player ($50.00 discount if paid in full by August 1st)

U8-U13 Boys & Girls $650.00 (For returning travel players only)

*Every player selected to our program will need to make a $100.00 deposit on June 9th or June 10th at our registration night. If you have any questions regarding the forms, please contact the registrar, Mirlene Dubreuze, at 954.494.6419 between the hours of 5pm and 8pm.
